There were 5 unrelated children found in care at the time of inspection.

Provider could not provide annual training hours during inspection.

An infant was observed sleeping in a swing.

The center director has been unable to meet director responsibility requirements. Repeated deficiencies in the areas of background checks and fire safety have occurred.

During the investigation and video, it was found that the caregiver did not demonstrate a high level of judgment or self-control.

The staff stated that the permit holder is aware of the physical state that the classroom walls and floors are in. They stated that they are no longer accredited as a result and that work orders have been put in, but no repairs have been completed.

Based on the information gathered during the investigation, it has been determined that a child was left unattended, the caregiver was not at a physical proximity that allowed visual and/or auditory awareness of the child in care.

The operation knowingly allowed persons to be present at the operation before receiving notification from the CBCU that the persons eligibility determination is Eligible, Provisional or Eligible with Conditions.
