A child was left unsupervised.

Based on the information gathered during the investigation, it has been determined that a child was left unattended, the caregiver was not at a physical proximity that allowed visual and/or auditory awareness of the child in care.

Two infants did not have current infant feeding schedules.

Two caregivers were observed administering prohibited punishment. One caregiver was observed pulling a child by the arm from underneath a table and another caregiver was observed grabbing a child by the arm and holding onto the child to keep the child from moving out of a chair. The same caregiver was observed grabbing on a child and pulling on the child's arms.

During the investigation and video, it was found that the caregiver did not demonstrate a high level of judgment or self-control.

A 18 mo old was observed to be sitting in a highchair upon arrival. A volunteer was observed removing the child from the chair and telling them they could not hit and not to hit again. The caregiver of the gorup initally stated the chair is only used for eating. After being informed of the volunteer's statement, the caregiver then stated it is used but for only 1 minute.

Two child files were found to missing the required hearing and vision.

One out of four staff did not have the required 30 hours of training. Staff member was missing training in child maltreatment, SIDS, shaken baby, early brain development, administering medication, food allergies, building and premises safety, hazardous materials, and emergency preparedness. Staff was also missing trainings in sections (a-f) of this standard. One out of four staff did not have the required 24 hours of annual training. The staff member was missing training in child maltreatment, communicable diseases, administering medication, food allergies, building and premises safety, hazardous materials and blood borne pathogens, and emergency preparedness. Staff was also missing trainings in sections (a-f) of this standard. One out of four staff required pre-service training. Staff member did not have the required 8 hrs. of pre-service training before having unsupervised access to children and did not have the required 24 hours of training within 90 days of employment. The staff member is missing all training in sections (a-f) of this standard.
