During a review of the staff trainings there were trainings certificates with the director's name on it. The director was listed as the trainer.

A child did not have their hands washed following a diaper change.

This standard was reviewed in relation to the allegations and found to be deficient. The operation did not report a caregiver who was arrested to licensing in the appropriate time frame.

During the inspection a caregiver was observed to be alone with a group of children when the conditions of their background check stated that they cannot be alone with a child or group of children.

Re-cite: Documentation of drills was not available for review at the time of inspection.

During inspection, the schoolers classroom was observed with Netflix on that was not educational and did not correspond to the daily activities.

The operations classrooms are out of ratio at times.

Based on the information gathered during the investigation, it has been determined that a child was left unattended, the caregiver was not at a physical proximity that allowed visual and/or auditory awareness of the child in care.
